Exceptional Family Member Program (EFMP) for Active Duty Sponsors

Services provided for Active Duty sponsors who have family members with special medical or educational needs. These services are provided offered at the JBSA Installations, Ft. Sam Houston, Lackland Air Force Base and Randolph Air Force Base.
Who is an Exceptional Family Member (EFM)?
An Exceptional Family member (EFM) is a Family member (child or adult) with any physical, emotional, developmental, or intellectual disorder that requires special treatment, therapy, education, training or counseling. Some examples of common diagnosis for EFMP enrollment are (list not all inclusive)
ADD/ADHD/ODD
Allergies (if require allergy shots or follow up with an allergist more than once a year
Asthma/RAD
Autism Spectrum/Pervasive Developmental Disorders/Developmental Delays
Cancer (unless treatment is completed, in remission >5 years, and no follow up)
Cervical Dysplasia/Abnormal Pap Smear
Diabetes
Neuro/Mental Health Disorders (Anxiety, Bipolar, Depression, OCD, PTSD, etc.)
Ostomies/Shunts/Prosthetics (artificial openings & artificial body parts)
Premature/High Risk Infants (<36 weeks, require extended hospital stay)
Special Ed/Early Intervention Req. (services required per Individual Support/Education Plan)
DD Form 2792-1 to be completed by Early Intervention Program or School. Substance Abuse (need for Drug and Alcohol Counseling Services) any other medical, psychological or educational condition should be considered if follow up by a specialist or special services are required.
