Transition Planning & Services
-
Welcome to the JISD Transition Planning and Services webpage! Transition Services involves thinking about the future and figuring out interests, preferences, strengths and setting goals for life after high school (education and/or training, independent living, community participation
etc). With these goals in mind we design the school experience to include both academic and functional skill development including employability and self-advocacy/determination skills, and agency linkages to promote movement toward long range goals. With this in mind and because transition planning can be complex we have developed this page with a wealth of information so we can partner to develop the most appropriate plan and implement transition services to facilitate movement towards the students postsecondary goal. Please feel free to contact the transition coordinator with any questions.
