Required Documents

  • To complete enrollment for Pre-K programs, original documents must be taken to the campus for verification.  Below is a specific list of required documents. All students ages 3 or 4 years ( by September 1st) qualify for the program. Certain forms might be needed to provide additional resources for your child and for reporting purposes. 

    Required Documents:

    • Birth Certificate
    • Proof of Address
    • Parent ID
    • Immunization Record
    • Home Language Survey (provided by school)
    • Proof of Income

    Additional Documents if applicable:

    • Documentation related to the foster care system
    • Documentation related to military service
    • Tax returns or paystubs
    • Copy of the resolution (certificate) for the Star of Texas Award
