CEP Verification Forms: Your Participation Is Needed At 13 JISD Schools

Community Eligibility Provision Information

Judson ISD needs your help in its participation in the Community Eligibility Provision (CEP) Program.

13 campuses are a part of this program that provides free breakfast and lunch meals to children under the federal Healthy, Hunger-Free Act for the National School Lunch and School Breakfast Program for school year 2019 – 2020. That means students at these campuses receive free breakfast and lunch while they are in school.  But we need your help in filling out a Verification Worksheet that is used for student economic status information. This information is completely confidential and is extremely important in helping this breakfast and lunch program to continue by submitting accurate information.

The Schools (13) that qualify for the Community Eligibility Provision are Kirby MS, Candlewood ES, Converse ES, Ed Franz ES, Elolf ES, Hartman ES, Hopkins ES, JCARE, Miller’s Point ES, Paschall ES, Park Village ES, Spring Meadows ES, and Woodlake ES.

These paper forms have been distributed by the campus listed above for you to complete and return, but for your convenience, there is also an electronic form to print and complete in English or Spanish if you wish. Just download it by clicking on the link below, fill it out and return it to your campus. We are very appreciative for your help.
